Terms & Conditions – Customer Survey Promotion

Information on how to enter and prizes form part of these Terms and Conditions. By entering this promotion, entrants agree to be bound by these Terms and Conditions. 

  1. Promoter

The Promoter is Team Medical (ABN 30 150 132 504) of PO Box 646, Seven Hills, NSW 1730 (“Promoter”). 

This promotion is conducted in accordance with all applicable State and Territory trade promotion legislation. Where required, the Promoter will comply with relevant regulatory requirements. 

  1. Eligibility

Entry is open to Australian residents aged 18 years or over who have purchased from Team Medical within the 12 months prior to the commencement of the promotion. 

Directors, management and employees of the Promoter and its related bodies corporate, and their immediate family members, are ineligible to enter. 

The Promoter reserves the right to verify eligibility, including proof of purchase history. 

  1. Promotion Period

The promotion commences at 9:00am AEDT on Monday, 9 March 2026 and closes at 6:00pm AEST on Friday, 10 April 2026 (“Promotion Period”). 

Entries received outside this period will not be accepted. 

  1. How to Enter

Entry is free. 

To enter, entrants must during the Promotion Period: 

  • Complete the customer survey via the official survey link; and 
  • Provide their full name, phone number and email address. 

Only one (1) entry is permitted per person. 

Incomplete, illegible or indecipherable entries will be deemed invalid. 

  1. Prizes

There are three (3) prizes to be won. 

The total prize pool is valued at $1,000 AUD (including GST) and consists of: 

  • 1 × $500 Prezzee voucher 
  • 2 × $250 Prezzee vouchers 

Prizes are not transferable, exchangeable or redeemable for cash. 

If a prize becomes unavailable for any reason, the Promoter reserves the right to substitute the prize with another prize of equal or greater value. 

Additional Prize Conditions 

Prezzee vouchers are issued by Prezzee Pty Ltd and are subject to Prezzee’s terms and conditions of use, including any expiry periods, redemption conditions and merchant restrictions. Winners are responsible for reviewing and complying with those terms. 

The Promoter is not responsible for any loss, damage or inability to use a voucher due to the acts or omissions of Prezzee Pty Ltd or any participating retailer. 

  1. Draw and Winner Selection

The draw will take place at 12:00pm AEST on Monday, 13 April 2026 at:

Three (3) winners will be randomly drawn from all eligible entries. 

The Promoter’s decision is final and no correspondence will be entered into. 

  1. Notification of Winners

Winners will be notified by telephone and/or email within two (2) business days of the draw. 

Winners’ names will be published on the Promoter’s website at www.teammedical.com.au on Friday, 24 April 2026. 

Prizes will be delivered as agreed between the winner and the Promoter. 

Winners must claim their prize within three (3) months of the date of notification. 

  1. Unclaimed Prize Draw

If any prize remains unclaimed three (3) months after notification, a redraw will take place at 12:00pm AEST on Tuesday, 14 July 2026 at:

The redraw winner(s) will be notified by telephone and/or email within two (2) business days and published on www.teammedical.com.au. 

  1. Verification

The Promoter reserves the right to request verification of identity, residential address, proof of age and proof of eligibility (including purchase history). Failure to provide requested documentation may result in disqualification. 

  1. Liability

Nothing in these Terms and Conditions excludes, restricts or modifies any consumer rights under the Australian Consumer Law. 

Except for any liability that cannot be excluded by law, the Promoter excludes all liability for any loss, damage or injury suffered or sustained (including indirect or consequential loss) arising in connection with the promotion or the acceptance or use of any prize. 

The Promoter is not responsible for late, lost, misdirected or incomplete entries due to technical disruptions or otherwise. 

  1. Social Media

If this promotion is communicated via any social media platform (including but not limited to Facebook, LinkedIn or X), entrants acknowledge that: 

  • The promotion is in no way sponsored, endorsed, administered by or associated with the relevant platform. 
  • Entrants are providing their information to the Promoter and not to the social media platform. 
  • Use of social media platforms is subject to their terms of use. 

Any questions regarding the promotion must be directed to the Promoter. 

  1. Privacy

The Promoter collects personal information for the purpose of conducting this promotion, including contacting winners and publishing winner details as required by law. 

The Promoter may also use personal information for marketing and promotional purposes, in accordance with its Privacy Policy. 

Entrants may access, update or correct their personal information by contacting the Promoter on 1800 128 268.

  1. General

The Promoter reserves the right to disqualify any entrant who tampers with the entry process or breaches these Terms and Conditions. 

If for any reason this promotion is not capable of running as planned, including due to technical failure, unauthorised intervention, fraud or any cause beyond the control of the Promoter, the Promoter may cancel, terminate, modify or suspend the promotion, subject to any written directions from a regulatory authority. 

Disputes concerning the promotion will be resolved by the Promoter acting reasonably and in accordance with applicable legislation.
